The First Visit
All services at Heartland CARES, Inc. are for those who have tested HIV positive.
The intake is intended to provide support, education, and information to all
new clients, whether they are newly diagnosed or simply new to services at Heartland
CARES, Inc. All services are low cost to no cost, depending on the client's income.
A new client's first appointment usually takes about 2 ½ - 3 hours. To
make an appointment, call us at (270) 444-8183 or toll free (877) 444-8183. The
first appointment at Heartland CARES, Inc. involves meeting with the case manager,
physician, and other medical and support staff.
The staff in the front office schedules new client appointments and informs
the staff with whom the client will meet. Upon speaking with the client, the
case manager will instruct the client on what he or she needs to bring to this
first appointment. When the client arrives for the appointment he or she completes
some basic forms in the reception area.
Next, the client meets with the case manager to complete enrollment paperwork
and identify needs related to health insurance, access to medications, housing,
and other services. This first meeting with the case manager provides the client
with an opportunity to learn about services, determine eligibility for programs,
ask questions, and for the client and case manager to begin to get to know
each other.
Following the meeting with the case manager, the client will meet with various
other providers. These may include any combination of the following:

Mental Health Counselor
Registered Dietitian
Care Coordinator
Prevention Specialist
Registered Nurse
Adherence Counselor
Life Work Counselor

The physician will also complete a medical history and physical for each new
client at their first appointment.

each other to plan and coordinate care for each individual. It is a goal
of Heartland CARES, Inc. to help a new client feel welcome and comfortable.
Beginning
and continuing treatment can be challenging. We hope that new clients feel
