Case Management

The case management department at Heartland CARES, Inc. strives to provide client centered supportive services to all our clients as part of a Multi-Disciplinary Team. Each new client is offered the opportunity to meet with and be assigned to a Case Manager and/or Care Coordinator. The case manager and/or care coordinator can work closely with the client to improve quality of life by identifying needs and developing plans to meet the daily challenges of living with HIV.

All clients receive initial case management services upon enrollment with Heartland CARES, Inc. On the first visit, the client meets with the case manager to complete enrollment paperwork and identify needs related to health insurance, access to medications, housing, and other services. This first meeting with the case manager and/or care coordinator provides the client with an opportunity to learn about services, determine eligibility for programs, ask questions, and for the client and case manager and/or care coordinator to begin to get to know each other.

Clients receive individual psychosocial support services and facilitated group services. Support services focus on problem-solving in a one-on-one environment. Services are currently focusing on the needs of clients facing the challenges of taking complicated combination drug therapy regimens (for more information, go to adherence page), and assessment of clients in the process of adjusting to newfound health.
